And I also noticed na doon sa CR ko na may portion na 'previous CR no.', at filled out ito.
My guess is, since pareho 5 years yung difference nung atin (2002 vs 1997 and 1997 vs 1992), baka naman may Original CR at yung portion na date of 1st reg is actually referring to latest reg..

nagbabago ang form over the years.. baka na miss out lang yan
