I'd say the city's at fault for not marking the road or having signs to guide motorists.
There was probably room for the van if it went straight. But, it's at fault for getting into the sedan's lane. Should've gunned the engine to stay ahead and stayed at the fringes instead of veering into the next lane.
The innermost lanes are also almost opposite each other which would indicate a turning lane. But, the lanes are not clearly marked. Plus, there's room for three columns of cars across the intersection. Usually, if it's a pure turning lane, the other end across the intersection loses that lane. It doesn't in this case.
So, there's supporting evidence for the van to go straight. But, it still should've stayed on its lane. Follow the imaginary curve.

Alanganin din yun contour ng kalsada. Mali lang nung innova eh kumabig agad sya pakanan.. yung kotse dapat kumabig din pakanan pero diniretso nya..

pero with a LOT of caution, i usually stay on the right though; unless di talaga ako familiar sa area, nagkakamali din minsan. tulad nung nasa kalookan yata ako or valenzuela, bigla nalang ako nagulat may kakaliwa ako na kasalubong; intersection pala, babad kas jeep sa outer lane
