NHTSA Safety Advisory: Reducing crashes caused by pedal error | Automotive World

Apparently these pedal-confusion accidents happen 44 times a day in the USA, according to the NHTSA.
The driver groups most affected are the under 20 and over 65.

my personal suspicion is, "there's something wrong with the design of the driver ergonomics, making the mis-pedalment more prone".
it could be something as obvious as the positioning of the pedals, or something less obvious, like the positioning and height of the driver's seat, or etc etc.
in my view, it needs a serious study.
for example, "why the mitsubishi montero?"

There is actually a study conducted on that. Skimming through the first couple of pages it seems like the greater majority of pedal confusion accidents when stopped or slowing are caused by slipping onto the wrong pedal, rather than outright stepping on the wrong pedal from the start.
It's when turning that outright pedal misapplication happens more often, I guess because of increased mental load on the driver.

Interesting read. And I do think that maybe the A/T pedals are indeed too close to each other. SUA happens far less often with manuals where there is significant spacing between each of the three pedals.

They go on to explore other factors like driver height and foot size. Taller drivers tend to pivot their foot, whereas shorter drivers will have to lift their foot completely in order to step on another pedal.

It's like saying mobile phone camera designs should be studied ... because there are approximately 250+ deaths per year caused by taking selfies ... or maybe it's just because the users are mentally lacking and shouldn't be allowed to drive / take selfies ...

Google Audi 100 / Audi 5000 scandal ... the grandfather of unintended acceleration issue ...
